MDS coordinator
Capture supporting diagnoses in Section I only when the record shows active treatment, monitoring, or evaluation this look-back. Unsupported codes are both a recovery miss and an audit risk.
Director of nursing
Nursing notes should name the clinical condition, the intervention, and the response — not a generic “resident stable.” Auditors read the note, not the ICD list.
Therapy
Document skilled need, goals, and barriers each session. Functional scores that jump without a matching narrative are a common RAC/TPE flag.
